All NEW Clients MUST fill out the Client Agreement Form. This is REQUIRED BEFORE filling out a Request for Assistance form.
Once a COMPLETED Client Agreement Form has been submitted, you will submit a Request for Assistance form. Requests for Assistance forms without ALL requested information (as advised in the Client Agreement Form) will be deleted/rejected WITHOUT notice.
Once your Request for Assistance form has reviewed by a member of our team, you will be contacted via text message (if ALL requested information has been provided) to request an appointment time via our Facebook appointment scheduling system (MUST BE DONE ON A COMPUTER!).
APPOINTMENTS MADE WITHOUT BEING ADVISED TO DO SO WILL BE REJECTED.
BEFORE your scheduled appointment, you are required to confirm via text message with us that you are going to be coming to your scheduled appointment time. This MUST be AT LEAST 4 hours prior or your appointment time will be CANCELLED (as advised and agreed to in the Client Agreement Form).
